If you were ever the recipient of any institutional action by any college or medical school for unacceptable academic performance or a conduct violation, you must answer Yes to the question about institutional action. You need not disclose if the institution subsequently deleted or otherwise removed the institutional action from your academic record.
Your academic record may consist of an official transcript as well as a student conduct record. Your official transcript will likely include institutional actions for unacceptable academic performance. Your student conduct record will likely include institutional actions for conduct violations. Most institutional actions do not interrupt your enrollment or require you to withdraw.
If you are unsure whether you were the recipient of an institutional action, you may obtain copies of your official transcript and disciplinary or conduct-related records from the relevant institution’s registrar’s office or office of student conduct (or equivalent).
Failure to provide an accurate answer to the institutional action question in the AMCAS primary application or a school’s secondary application, if applicable, may result in an investigation.
Medical schools understand that many individuals learn from the past and emerge stronger as a result. Full disclosure will enable medical schools to evaluate the information more effectively within the context of your application.
If you become the subject of an institutional action after certifying and submitting the AMCAS application, you are required to inform your designated medical school(s) within 10 business days of the date of the occurrence.
If you select Yes, you were a recipient of an institutional action, you will be prompted to select an institutional action category from the drop-down list (“academic,” “conduct,” or “both”). Examples of academic institutional actions include, but are not limited to, academic probation and academic standing warnings. Examples of conduct institutional actions include, but are not limited to, residence hall policy and student code of conduct policy violations. Select “Both” if you received an academic institutional action and a separate conduct institutional action.
If you are reapplying and previously answered Yes to the institutional action question on a prior application, answering No on this application will trigger an investigation. As part of the investigation, you may be asked to have the institution provide documentation directly to the AMCAS program confirming that a prior action was deleted or otherwise removed from your record.
