The Fee Assistance Program application asks for general information and financial information about you, your spouse (if applicable), and all of your living parents (if applicable). 

Before starting your Fee Assistance Program application, please gather the following information and documents to help ensure a smooth submission:

1. Personal Information

  • Legal name (as it appears on your documentation).
  • Date of birth.
  • Current mailing address and contact information. (Please see proof of a US-based home address requirement.)

2. Tax Information

  • Most recent federal tax return (IRS Form 1040), if filed
  • W-2s, 1099s, or other income documents, if applicable
  • If you did not file taxes, be prepared to enter other forms of income.

3. Parent Information (If Required)

  • Parent(s)’ tax information and income details, if applicable based on your age 
  • Documentation explaining special circumstances, if required

4. Income and Financial Resources

  • Current income information
  • Financial support received from others (if applicable)
  • Benefits or assistance received (if applicable)

5. Household Information

When determining your household (family) size, please follow these guidelines:

  • Count 1 for yourself.
  • Add 1 for your spouse (if applicable).
  • Add 1 for each dependent listed on your IRS 1040 tax form.

Your household size (also referred to as family size in the Fee Assistance Program Essentials Guide) is based on individuals claimed on your tax return — not simply who you live with.

If you are not married and do not claim any dependents, your household size is likely 1, even if you live with siblings, extended family members, or others.

If you are married and do not claim any dependents (as determined by your IRS 1040 tax form), your household size would be 2 — you and your spouse.
 

6. Supporting Documentation

  • Required documentation is based on the information you provide in your application.
  • If you indicate that you received specific types of income, you will be prompted to upload supporting documentation.
  • Documents must be clear, complete, and current for the applicable tax year.

7. Time to Complete the Application

  • Approximately 20–30 minutes to complete, if documents are ready
  • Additional time may be needed if supporting documents are required

Helpful Tips

  • Review all entries for accuracy before submitting.
  • Any document that requires a signature must include a handwritten or verified digital signature (e.g., DocuSign or Adobe). Typed signatures are not accepted.
  • Respond promptly if additional documentation is requested.
  • Monitor your application status regularly after submission.
