Initial |
An Initial application is your first request for accommodations. You must complete an Initial request for accommodations before you will be able to submit any of the other types of requests for accommodations. You may also be asked to complete a new Initial application if it has been more than 5 years since your last determination. |
Reconsideration |
A Reconsideration request is a request to have your application re-reviewed or a request for additional accommodations that have not been previously requested. It should include new and substantial information. |
Appeal |
An Appeal request is a request to have your application re-reviewed without any additional or substantially different documentation to provide. It may only be submitted once, and determinations are final. Carefully consider whether a Reconsideration or an Appeal is the best option for seeking a re-review of your accommodations request. |
Extension |
An Extension request is a request to extend approval of only your previously approved accommodations. If you are seeking additional accommodations in addition to an extension of previously approved accommodations, this should be requested via the Reconsideration process. |
Incomplete Application |
If your application has been deemed incomplete, it does not provide sufficient information to allow for a full review of your request. This is not a denial. You will not be able to submit a Reconsideration or Appeal request until an incomplete Initial application has received a determination. |
