Transcripts
- For ADHD, Learning Disabilities, Psychiatric Disabilities, & Vision requests, official transcripts from high school, undergraduate and/or any graduate level institutions attended are required.
- For Physical requests (except Vision), transcripts are recommended. Please be aware, depending on the nature of the functional limitations and requested accommodations (i.e., extended time), you may be asked to submit transcripts.
- Scanned official or unofficial transcripts will be accepted.
Standardized Testing Scores
- For ADHD, Learning Disabilities, Psychiatric Disabilities, & Vision requests, all previous standardized exam score reports such as SAT, ACT, PSAT, GRE are required.
- For Physical requests (except Vision) previous standardized scores are recommended. Please be aware, depending on the nature of the functional limitations and requested accommodations (i.e., extended time), you may be asked to submit score reports.
- Official copies are encouraged, unofficial copies will be accepted.
- If are you unable to obtain previous standardized exam scores, please provide an explanation on related questions in your application and/or your Cover Letter to be submitted.
Documentation of Accommodations Received
- For ADHD, Learning Disabilities, Psychiatric Disabilities, & Vision requests, verification of accommodations received (if applicable) from high school, undergraduate and/or any graduate level institutions attended are required.
- For ADHD, Learning Disabilities, Psychiatric Disabilities, & Vision requests, verification of accommodations received (if applicable) on all previous standardized exams such as SAT, ACT, PSAT, GRE are required.
- If applicable, we strongly encourage examinees to submit additional documentation which may help establish a history of academic accommodations received, such as Individual Education Plans, 504 Plans, College Disability Services Plans.
- When considering requests for modifications, accommodations, or auxiliary aids or services, the AAMC gives considerable weight to documentation of past modifications, accommodation, or auxiliary aids or services received in similar testing situations, provided in response to an Individual Education Program provided under the Individuals with Disabilities Education Program, or a plan describing services pursuant to section 504 of the Rehabilitation Act of 1973.
Supporting Academic Records
- Documentation may include, but is not limited to, notes on unofficial accommodations received, tutoring evaluations, teachers' comments to substantiate self-reported past and current academic difficulties, elementary school progress reports and/or report cards, clinical notes or documentation from pediatrician or other treating source.
- Please be aware, depending on the nature of the functional limitations and requested accommodations (i.e., extended time), you may be asked to provide additional historical information.
Previous Evaluation Reports
- Previous evaluations will be used to understand the nature and history of your condition and the associated functional limitations.
