| Program Description: SLRP is an incentive program for primary care providers working at approved practice sites in Health Professional Shortage Areas (HPSAs). |
| Eligible Specialties: Primary care physicians, Nurse practitioners, Physician assistants, Dentists, Dental hygienists, Certified Nurse-midwives, Mental health or Behavioral health professionals, Registered nurses, and Pharmacists |
| Service Requirements: Commit to service obligation of at least two years. One year extension may be awarded for up to three additional years, for a maximum service obligation of five years. |
| Deadline/Application Cycle: Applications are accepted on a first come first serve ongoing basis. SLRP does not have a set closing date. Once all program funds have been exhausted this website will be updated and SLRP applications will be removed. |
| Amount: Qualifying providers can receive a maximum award of $35,000 per year of 25% of total debt, whichever is smaller. |
